Inspector Notes
An unannounced, on-site focused inspection was initiated on 06/17/2025 and completed on 06/17/2025, as a part of probation. The Department issued a Special Order on 05/12/2025 to place licensee on probation and require licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations.
The on-site inspection began at 9:50am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 32 children present and nine staff. This inspection included document review, tour of the facility, and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
