Inspector Notes
A Subsidy Health and Safety inspection was conducted on 04/02/2024 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review ad interview it was determined the provider and assistant provider did not complete the department's annual health and safety update course.
