Inspector Notes

An unannounced, on-site monitoring inspection was completed on 07/22/2025, as a part of the licensure period. The on-site inspection began at 11:45 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 2 staff. The inspector reviewed 3 children?s records and 2 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Poisonous materials shall be stored in an area inaccessible to children.

Cleaning chemicals were in an unlocked cabinet under the sink in the bathroom that the children use.

Documentation of additional immunizations is required once every six months for children under the age of two years. '

The records for the three children in care did not contain immunization updates every six months until the age of two years.
