Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on 9/27/24. The on-site inspection began at 11:10am and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 36 children present and 8 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/17/24. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center is required to ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

In the 3-year-old classroom there was a broken step stool at the sink. The door to the bathroom was broken at the hinge and it dragged on the floor. The door was difficult to move and could not be opened completely.

In the 2-year-old classroom, the bathroom had torn linoleum and peeling paint along the base boards and above the back of the toilet tank.

On the preschool/school age playground there were 5 tricycles with rust on the wheels.

There was peeling paint and rotting wood accessible to children on the white building on the playground.

There was a large dead branch hanging over the swing set and play area. The branch hung approximately 5 feet 7 inches from the ground. The branch swayed when touched and was accessible to school age children.

There was a broken board with the nail heads protruding from the white fence on the playground.

On the 2-year-old playground there was chipped red paint on the swing set and rust and cracked plastic on a swing seat and safety attachments.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the four-year-old classroom there were 2 cans of shaving cream in an unlocked closet.
There was a container fragrance oil on a shelf in the classroom. Both containers had warning labels that read, ?Keep out of reach of children,? and were within reach of children.

Based on observation, it was determined that the center did not ensure that where playground equipment is provided resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials F1292-99 (Compressed Loose Fill Synthetic Materials Depth Chart) and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

There was insufficient resilient surfacing on the 2-year-old playground. Grass and weeds had grown thickly into the sand and covered the fall zones in front, behind and on both sides of the swings. Grass was also growing in clumps beneath the swings.

A Little Tykes slide, approximately 40? high, was in the grass on the 2-year-old playground. There was no resilient surfacing under, on all four sides and in front of the slide chute.

On the preschool/school age playground the metal ground support and bolts for the small yellow slide were exposed and posed an injury hazard.

The center shall maintain, in a way that is accessible to all staff who work with children, a current list of children?s allergies, sensitivities and dietary restrictions. This list shall be kept confidential in each room or area where children are present.

The allergy list in the 3-year-old and he 2-year-old classroom was posted on a wall. The information could be seen by any person entering the classroom and was therefore, not confidential.

The first aid kit on the van used to transport children to and from school did not contain scissors, gauze pads, antiseptic cleansing solution, a thermometer, triangular bandages and single use gloves.

Any vehicle used by the center for the transportation of children shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of the children.
