During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 6/12/25 and completed on 8/15/25 in response to a complaint received by the licensing office on 5/30/25 relating to allegations of playground safety and supervision. The inspector reviewed 4 children?s records and 2 staff records onsite on 6/12/25, 7/24/25 and 8/11/25. This inspection included document review, tour of the facility, interviews, observations and measurements.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Though not a part of the original complaint, violations that were not reported were found related to transportation and records. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/29/25. A POC submitted after this date will not appear on the public website.
Violations
7The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition.
1.There was a stationary car on the playground; a piece of the metal frame on the bottom of the front passenger side was bent and had a sharp jagged edge. The car had rust, peeling paint, a broken tire and a missing tire.
2.The back of a bucket swing seat on the two-year-old playground had a large crack in it.
Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. The slide on the small playground was placed on grass and did not have resilient surfacing on all sides of the fall zones. There was grass growing in the fall zones around the small and large playground swing equipment.
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all of the required components. An injury report dated 5/22/25 did not contain future action to prevent recurrence of the injury and two signatures.
The center shall ensure that during transportation of children, Virginia state statutes about safety belts and child restraints are followed as required by the Code of Virginia, to assure the safety of children. Nine children under the age of eight years old were observed being transported in the facility van without booster or car seats.
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all of the required information. On 6/12/25 there was no record with the required information for child #2 who had been in attendance for one week.
