During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 11/6/24. The Department issued a Special Order to place licensee on probation for a period of six (6) months and require the licensee to contact parents, guardians, or other responsible persons (families) regarding health and safety violations on 4/8/24 as a result of high-risk violations. The on-site inspection began at 10:05am and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 66 children present and 21 staff. The inspector reviewed six children?s records and six staff records onsite. This inspection included document review, a tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
