Inspector Notes
An unannounced monitoring inspection and self reported injury investigation were conducted on 11/2/23 at 10:15am. There were 42 (8mos to 4yrs) directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, and playing outside. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
5Evidence: reviewed 5 program leader records and found that Staff 2, 3, 4, and 5 did not have documentation of program leader qualifications.
Evidence: observed that there was a tube of Neosporin marked, keep out of reach of children, accessible to children.
Evidence: observed small "lego-like" blocks that were small enough to be a choking hazard for children under the age of 3, in the Twos' classroom.
Evidence: observed that there were two bottles of diaper cream, in children's cubbies that were not labeled with either child's name.
Evidence: reviewed the facility's evacuation drill and interviewed staff and verified that there was no evacuation drill practiced in September of 2023.
