During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 6/10/25. The on-site inspection began at 1:10pm and ended at 2:55pm. The inspector reviewed compliance in the areas listed above. There were 64 children present and 11 staff. The inspector reviewed 6 children's records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 1, who had been enrolled for over a month, did not have documentation of an immunization record on file.
Plan of Correction: The center responded with the following: Child 1's parent will be contacted and the immunization record will be obtained and placed in the child's record.
Standard 8VAC20-780-140-A
Repeat Violation
The center shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no documentation of a physical examination in the record for child 1, who has been enrolled for over one month.
Plan of Correction: The center responded with the following: Child 1's parent will be contacted and documentation of the physical will be obtained and placed in the child's record.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff 1's updated TB screening was due one year ago and has not yet been completed.
Plan of Correction: The center responded with the following: Staff 1 will obtain an updated TB screening. Results will be placed in the staff record. Management will ensure that all staff obtain updated TB screenings within the required timeframe.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for child 1, enrolled for over a month, does not contain documentation of viewing proof of the child's identity and age.
Plan of Correction: The center responded with the following: Child 1's parent will be contacted and the missing information will be obtained and placed in the child's record.
