Inspector Notes

An unannounced renewal inspection was conducted on 1/4/23. There are 42 children present at the center with 7 staff. 13 children are attending school at the center. The center has 3 preschool rooms all in ratio. The children are observed playing on the blacktop, playing on the playground and later doing work individually and in small groups inside.
Lots of materials to explore and space for play. A violation notice was given to the center along with this inspection summary.

Based on the review of children's records the center failed to get immunization records for a child.
Evidence: Based on the review of 5 children's files, one child, child 1, is missing immunization records.
Plan of Correction: We have asked the parents several times for the immunization records. We will ask the parents again.

Based on the review of records, the center failed to get a physical for one child.
Evidence: Based on the review of 5 children's records the center failed to get a physical.
Plan of Correction: We have asked the parents several times for the physical record. We will ask the parents again.

Based on the observation at the beginning of the inspection, A grouping of children were out of ratio at the beginning of the inspection.
Evidence: Based on observation,at 10:40 a, when the inspector arrived, one teacher was present with 26 children ages 3- school age. The children were playing on the blacktop.The school age teacher had gone inside and the other teacher had taken a child to the bathroom.
Plan of Correction: One staff took a child in to use the bathroom and the child refused to come back out and the school age teacher went in to take something inside. I will make sure we talk about it and it does not happen again. The staff will take children in so we are in ratio.

Based on the review of emergency drills the center failed to conduct an emergency drill each month.
Evidence: The center failed to conduct an emergency drill for December 2022.
Plan of Correction: December got away from us, we were just here until 12/16. We will make sure we do monthly drills.
