Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/2/24 and completed on the same date, as a part of licensure period. The on-site inspection began at 11:20 and ended at 13:35. The inspector reviewed compliance in the areas listed above. There were 43 children present and 7 staff. The inspector reviewed 7 children?s records and 3 staff records on site. This inspection included, document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Staff must have subsequent background checks done every 5 years.

Staff 2 has background checks that must be redone, last background checks are more than 5 years old.
Plan of Correction: We will create a spread sheet so we can ensure that background checks are done every 5 years and that we submit the month of expiration.
Standard 8VAC20-780-160-C
Every two years a staff must have a subsequent TB screening done.

One staff, staff 1 is missing a subsequent TB screening.
Plan of Correction: We will send the staff to get a new TB screening.
Standard 8VAC20-780-245-A
Staff must complete 16 hours of training annually.

Staff 1 and Staff 2 are missing training hours, Staff 1 completed 5.5 hours in the last year and Staff 2 completed 3 hours in the last year.
Plan of Correction: We will create a better training schedule and also check all staff files to ensure that all staff have 16 hours of training.
