Inspector Notes

An unannounced, on-site Monitoring inspection was initiated and completed on 9/18/2025 , as a part of the licensure period. The on-site inspection began at 11:20AM and ended at 12:41PM. The inspector reviewed compliance in the areas listed above. There were 34 children present and 9 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The following record shall be kept for each staff person: Background checks as required regulation 289.035 B-4 Out of State Background Checks.

Staff #3 who has been employed for 30 months does not have evidence of completed out of state background checks.
Plan of Correction: We will make a request for the missing background checks today.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #3 ( Age 2 years) does not have an immunization record on file.
Plan of Correction: We will request updated immunizations from the family.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Preschool/School-age classroom did not have cabinet with cleaning supplies locked in the boys bathroom.
Plan of Correction: Corrected onsite - lock placed on cabinet.

The center shall meet the proof of child identity and age requirement within 7 days of the child attending the facility.

Child #3 (age 2 years) has been enrolled for 30 days and has no proof of identity on file.
Plan of Correction: We will request a copy of the child's birth certificate today.

When food is brought from home the food container must be clearly dated.

10 out of 17 lunchboxes checked were not labeled with a date.
Plan of Correction: We will remind families to label lunchboxes with date and follow up daily.
