8VAC20-780 Staffing and Supervision.
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 16 2025 and completed on October 22 2025, in response to a self-report relating to Staffing and Supervision. The on-site inspection began at 11:45am and ended at 10:30am on October 22 2025.. The inspector reviewed compliance in the areas listed above. There were 52 children present and 12 staff. The inspector reviewed 1 children?s records and 2 staff records. This inspection included a document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on November 7 2025. A POC submitted after this date will not appear on the public website.
Violations
1The center failed to provide care and protection for a child outside on October 10 2025. At approximately 5:22pm on October 10, 2025, there were 13 children outside with 2 staff members. Staff #1 stated that they were discussing a situation with a parent and Staff #2 was interpreting for them. Staff #1 stated that while speaking with the parent, they observed Child #1 ( 5 years of age) climbing an outdoor play furniture (54 inches tall) and climb onto the school building roof (approximately 72 inches tall). Staff #1 stated that they asked Staff #2 for assistance and then Staff #2 climbed onto the roof after the child. Staff #2 stated that they and Child #1 were on the roof for approximately 15 to 20 seconds and that Child #1 was never out of sight. Staff #2 stated that when they reached the roof, they were able to hold hands with Child #1 and help guild them back down to Staff #1 on the ground.
