Inspector Notes

A monitoring inspection was initiated on 4/11/24 and concluded on 4/11/24. There were 16 children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records were reviewed. The inspection started at 9:20 am and concluded at 10:35 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record reviews the center failed to ensure each center keep at the center documentation of viewing proof of the child's identity and age.
Child # 1 began care 8/14/23, there is no documentation of proof of the child's identity and age available.
Plan of Correction: Staff will ensure that all the required information is in the file.
