5-2-22 An unannounced monitoring inspection was conducted today.
Sixteen children were present with two staff.
The children were playing outdoors. Child and staff files, the physical plant, supervision and emergency procedures were reviewed.

Based on a review of the physical plant, the facility failed to ensure that all hazardous substances are kept in a locked place.
Evidence:
1. There is a large bottle of carpet cleaner on the laundry sink in room 2.
2. The cleaning supplies cabinet is not locked.
