An unannounced renewal inspection was conducted today.
There were 13 c hildren (ages 3-5yrs) present with two teachers.
Three child records and two staff records were reviewed.
The children were playing outdoors.

Based on observation during outdoor time, the facility failed to ensure that all areas operate in a safe condition.
Evidence:
1. Landscape fabric is exposed in multiple areas on the playground. This presents a tripping hazard.
