4-20-22 An unannounced monitoring inspection was conducted today.
Nine children were present with two staff. The children were eating lunch and playing outdoors.
Child and staff files, the physical plant, emergency evacuation records and nutrition were reviewed.

Based on a review of staff files, the facility failed to ensure that staff sign a sworn statement prior to the first day of employment.

Based on a tour of the playground, the facility failed to ensure that all equipment is maintained in safe and operable condition.
Evidence:
1. At the top of the steps of the yellow slide, the plastic covering is torn away from the metal platform. This presents a tripping hazard and sharp plastic that could tear skin.
2. There are five bolts with significant rust on the under side of the deck on the yellow slide.
