The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
The gate next to the building would not adequately close which left an opening that children could push open and get out of.
Plan of Correction: -upgrades all fencing and extend the fenced in perimeter-Gathering bids on 2/6/26

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance.
On 1/7/26, staff #1 and #2 were outside on the playground with 16 children. Staff #1 left the playground. Child #1 found an opening in the fence surrounding the playground and used the opening to leave the playground. Child #1 walked down the sidewalk beside the school, to the school parking lot and then onto the road. An unidentified passerby stopped and picked up the child and returned the child to Staff #1.

-Reviewed policies & procedures w/ signature acknowledgement
-Updated playground checklist w/daily checks visible for office to review-immediately
-secured the opening-immediately
-Playground off limits until further notice

Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On 1/7/26, Child #1, three-year-old, left the playground area unsupervised. A passerby stopped and told the staff on the playground that a child was going toward the road. Child #1 walked to the school parking lot, approximately 294 feet from gate, and then to the road. A second passerby stopped and picked up the child in the middle of the road. Child #1 was out of sight and sound supervision for approximately five (5) minutes.
