During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated on 1/22/2026 and completed on 2/5/2026 with a local agency, in response to a self-report received by the licensing office on 01/22/2026 relating to Supervision. This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. REPEAT VIOLATION, SYSTEMIC DEFICIENCY
When staff are supervising children, they shall always ensure their, care, protection and guidance.
On 1/22/26, staff #1,2 & 3 were preparing the children to go to the lunchroom. All three staff and 14 children left the building. Child #1 remained in the classroom, unsupervised for approximately 10 minutes.
Plan of Correction: -Retraining on standards of conduct and active supervision

REPEAT VIOLATION, SYSTEMIC DEFICIENCY
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
Child #1, 4 years of age, was left in the classroom approximately 10 minutes alone.
