Inspector Notes
An unannounced inspection was conducted on 10/18/2023 in response to a complaint that was received by the licensing department on 10/11/2023 regarding allegations of forbidden actions. On 10/18/2023, the inspector was on site from approximately 10:15am until 11:30AM. Interviews with staff were completed, and observations were made. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Additional areas of noncompliance not related to the complaint were cited. See the violation notice for more details.

(Repeat Violations) Based on observation, it was determined that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.
