An unannounced inspection was initiated on and completed on September 12, 2024, in response to a complaint received by the licensing office on September 6, 2024, relating to allegations of forbidden actions. The on-site inspection began at 12:30 pm and ended at 1:10 pm. The inspector reviewed 2 staff records on-site. This inspection included document review, observation and interviews with staff.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
