Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 1/7/22. The director was contacted by telephone and a virtual inspection was conducted. There were 38 children present, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed, in addition to other center documents. Medications and authorizations were reviewed. Children were observed paying in small centers, eating snack and listening to a story.

Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child # 1's date of birth is 7/6/20.
2. Child # 1's most recent documentation of immunization was dated 2/2021. Additional immunization records were not obtained after that date.
Plan of Correction: The additional records will be obtained.
