Inspector Notes
An unannounced complaint inspection was initiated on 3/3/22 and concluded on 3/14/22. A complaint was received by the department regarding allegations in the areas of ratios, nap time and children's injuries. 5 staff were interviewed. Classroom observations during and after nap time were made. Child Supervision Records (CSR's) and injury reports were reviewed. The evidence gathered during the investigation supported the allegation related to ratios not being met at nap time. Violations not related to the complaint but identified during the course of the investigation are also listed on the violation notice. Violations
3Evidence:
1. Preschool (3 years old) classroom: a. The home living sink was missing the bin that would serve as the sink, posing a head entrapment hazard; b. One soft child sized couch was observed to have large rips on the seat exposing the internal material; c. The seat of the second soft child sized couch was found to be detached and ripped from where it connects to the back supporting part. The gap was wide enough that a child could insert their hand in that gap. The wood inside the couch was visible to the eye.
2. Pre-K (4 years old) classroom: The home living sink was missing the bin that would serve as the sink, posing a head entrapment hazard.
3. Chipped paint was seen in the Preschool and Pre-K classrooms around the windows.
Evidence:
1. On 3/3/22, in the Pre-K classroom (4 years old) there were 13 children present with Staff # 1.
2. At 2:16 pm, while nap time was still in session, a total of 8 children were awake and observed to be getting off their cots, walking around the classroom and moving over to other cots to talk to their peers.
3. At 2:22 pm, 12 out of the 13 children present were awake. Staff # 1 was the only member in the classroom at this time.
4. When asked by the licensing inspector if another staff would be coming to assist, Staff # 1 stated that she was not sure when but another staff member was expected to come in and help.
5. At 2:31pm, when Staff # 2 was informed by the licensing inspector that all children were awake, Staff # 3 was sent to the Pre-K classroom to assist Staff # 1.
Evidence: On 3/3/22, there were three occupied cots by the windows in the Preschool (3 yrs old) classroom that were not 12 inches apart.
