Inspector Notes

An unannounced monitoring inspection was conducted on 5/6/2024 from 2:00 pm- 4:00 pm. At the time of entrance, 36 children, ages 3 to 4 years old, were in care with 5 staff members present. The sample size consisted of 5 children?s records and 4 staff records. Children were observed washing hands, having an afternoon snack and playing outdoors. Medication administration was not observed. There are currently no medications stored on site to be administered to children in care. Children are not routinely transported in a vehicle while in care. However, fieldtrips may be scheduled on occasion requiring transportation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the licensee did not always ensure that each employee obtains the required fingerprint based criminal record check prior to employment.
Evidence:
For staff records were reviewed.
The record for staff #2, whose employment in the licensed program began in June 2021, contained result s of a criminal records search obtained through the State Police which was completed for her employment at the private school. However, the employee had not obtained the required Criminal Records Search through the Office of Background Checks (OBI) for her employment in the licensed before and after school program.
The program director confirmed that the OBI background check had not been obtained for staff #2.
Plan of Correction: Staff #2 will be asked to make an appointment to have her fingerprints taken for the Criminal Records Search within 10 days. The results will be forwarded to the licensing inspector upon receipt.
