In two separate classrooms, cots were closer than 12 inches. In Classroom C, there were 3 cots that were observed to be approximately 4 inches apart. In Classroom E, there were 9 cots in various locations of the classroom that were within 12 inches of each other.

Plan of Correction: Cots were rearranged and spaced to the needed 12 inches apart while the visit took place. Moving forward cot map allows 12 inches between all cots.
