Violations

1
Standard 8VAC20-780-450-A
Based on observation, the center did not ensure that cribs, cots, mats and beds used by children other than infants during designated rest periods should have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
1)In the Pre-K classroom (ages 4-5 years old), there were 9 children sleeping on mats without a bottom cover or without a one-piece covering open on 3 edges.
2)In the Preschool classroom (ages 2-3 years old), there was 1 child sleeping on a mat without a bottom cover or without a one-piece covering open on 3 edges.
Plan of Correction: Linen order placed on 5/29/24 for additional linens and in abundance for future compliance. Top and bottom linens utilized on 5/30/24 until delivery received.
