Inspector Notes

An unannounced inspection was initiated on July 3, 2024, and completed on July 22, 2024, in response to a complaint received by the licensing office on June 26, 2024, relating to allegations of administration, nutrition and food services, and preventing the spread of disease. The inspector reviewed one child?s record on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/04/2024. A POC submitted after this date will not appear on the public website.

Based on a review of documentation and interviews, the center did not ensure children?s records were treated confidentially.

Evidence: During interviews, it was reported that on June 25, 2024, the center posted confidential information pertaining to Child #1 on a refrigerator in the Infant classroom. The refrigerator is accessible to other parents of children enrolled in the classroom.

Upon review of the document that was posted, it was determined that the information included the child?s name, the child's parent's name, and information regarding a medical condition.
Plan of Correction: Per the Center: "Confidential communication regarding children and families will be limited to the main office."
