An unannounced monitoring inspection was initiated on 11/13/2024 and completed on 11/20/24, in response to a self-report received by the licensing office on 11/07/2024 relating to?a serious injury. The inspector reviewed one child?s record on-site. This inspection included document review, tour of the facility, interviews, and observations.
