During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 8/15/2025 and completed on 8/22/2025 with local agencies in response to a complaint received by the licensing office on 8/4/2025 relating to allegations of Supervision and Forbidden actions. The inspector reviewed two children?s records and five staff records on-site on 8/22/2025. This inspection included document review ,tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/4/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

Staff reported during interviews on August 15th and August 22nd, 2025, that they observed Teacher #1 yelling loudly and aggressively at the children in classroom A, which resulted in children crying.
Plan of Correction: All staff retrained on positive child guidance by 9/19/2025. Teacher #1 is not in the center.

The following actions or threats thereof are forbidden: Physical punishment, striking a child, roughly handling or shaking a child.

Staff reported during interviews on August 15th and August 22nd, 2025, that they observed Teacher #1, roughly handling children in Classroom A, which included grabbing children by the wrists quickly, forcefully placing them in chairs and pulling children by the upper arms or picking them up under the arms in a rough manner.
Plan of Correction: All staff retrained on positive child guidance by 9/19/2025. Teacher #1 is not in the center.
