Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on December 6, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 63 children present, ranging in ages from three months to five years, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Based on review of three children's records and interview, the center did not ensure documentation of a physical exam was obtained prior to or within the first 30 days of one child attending the center.

Evidence:
The record for Child #1 (started attending center on 06/07/2021) did not contain documentation of a physical exam. A member of management confirmed this information.
Plan of Correction: We have previously requested this information. We will let the family know this information is needed to complete the child's record.
