Inspector Notes
A renewal inspection was conducted Tuesday, April 18, 2023. The licensing inspector was onsite, completing the inspection from approximately 10:30am until 1:00pm. There were 40 children present, ranging in ages from two months to five years, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of seven children's records and six staff members? records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

The records for Child #2 (first date of attendance 09/12//2022) and Child #5 (first date of attendance 04/10/2023) did not contain documentation of immunizations. A member of management confirmed this information during the inspection.

The paint on two red doors on the storage units located on the playground was peeling. A member of management stated the maintenance team has already been contacted and requested to fix the doors.
Evidence:
1. In the Four?s classroom, three electrical outlets located on the wall did not contain protective covers. Twelve children were observed in the classroom.
2. In the Three?s classroom, one electrical outlet located on the wall did not contain protective covers. Eleven children were observed in the classroom.
3. In the Two?s classroom, one electrical outlet located on the wall did not contain protective covers. Eleven children were observed in the classroom.
