Inspector Notes

A monitoring inspection was conducted January 22, 2024. The licensing inspector was onsite, completing the inspection from approximately 12:30pm until 2:30pm. There were 39 children present, ranging in ages from three months to five years, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and five staff members? records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Based on review of five staff records and interview, the center did not ensure one staff member completed the Virginia Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
1. The records of Staff #5 (Date of Employment 10/20/2023) did not contain documentation of completing the Virginia Department of Education-sponsored orientation course within 90 days of employment.
2. A member of management stated Staff #5 has begun the training but has not completed it as of 01/22/2023.
Plan of Correction: The staff member has begun the training and we have it completed by the end of this week.
Standard 8VAC20-780-70
Based on review of five staff records and interview, the center did not maintain the required documents for one staff member.
Evidence:
1. The records for Staff #4 (Date of Employment 06/05/2023)did not contain documentation of staff completing required orientation.
2. A member of management stated orientation was completed with Staff #4, but the form could not be located.
Plan of Correction: The orientation was completed with the staff member but the form could not be located during the inspection. We will continue to search for the form or complete a new form.
