Inspector Notes

A Subsidy Health and Safety inspection was conducted on 3/2/2023 to review supplemental health and safety requirements. There were 59 children present with 16 staff supervising.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard 8VAC20-790-600-D-1
Based on a review of six (6) staff records and interview, the vendor did not ensure that two (2) staff had a current CPR certification appropriate to the ages of children in care.

Evidence:
1. The record of staff #1 (date of employment: 8/28/2009) contained an expired certification. The record of staff #3 (date of employment: 9/12/2022) did not contain a current CPR certification within 90 days of employment.
2. The vendor acknowledged that the two staff were not CPR certified.
Plan of Correction: staff members without valid certification will obtain it upon testing day which is currently scheduled for 3/7/2023

Based on a review of six (6) staff records and interview, the vendor did not ensure that two (2) staff had a current first aid certification appropriate to the ages of children in care.

Evidence:

Based on a review of six (6) staff records and interview, the vendor did not ensure that one (1) staff annually completed the department's health and safety update course as required.

Evidence:
1. The record of staff #1 (date of employment: 8/28/09) did not contain a 2022 health and safety update.
2. The vendor acknowledged that staff #1 had not annually completed the training.
