Inspector Notes

An unannounced inspection was initiated on July 7, 2025 and concluded on August 15, 2025 with local agencies in response to a complaint received by the licensing office on June 30, 2025 relating to allegations of injury reporting. The inspector reviewed 2 staff records on-site. This inspection included document review, a tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation: therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to training and are documented on the violation notice issued to the program.

At least one staff in each grouping of children shall have current cardiopulmonary resuscitation (CPR). Staff 1 was alone with seven 2-year-olds each morning from approximately 7:00 a.m. until 9:00 p.m.. Staff 1 does not have documentation of current CPR with an in person competency component on file.
Plan of Correction: Staff will not be left alone in a grouping of children until CPR is completed.
