Inspector Notes

An unannounced monitoring inspection was initiated and completed 07/11/2022. The inspector was on site from 10:45 to 12:30.

There were 32 children ages ten months to five years of age, present under the supervision of six staff. Children were observed during outside play and naptime. Three children's records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violations notice issued to the program.

Based on observation the center failed to install carbon monoxide
detectors in each building that serves preschool children.

Evidence: Inspector asked if the center had carbon monoxide detectors installed and was informed they did not.
Plan of Correction: Maintenance will install by 7/16.
Standard 8VAC20-780-280-A
Based on observation, the center failed to keep hazardous substances in a locked place.

Evidence: Inspector observed cleaning materials on the shelves in classroom 1 and 2. The cleaning materials were out of the reach of children and there were no children present in the classroom at the time.
Plan of Correction: All chemicals were locked away immediately.

Based on observation, the center failed to provide addresses of emergency contacts for child?s record.

Evidence: Child #1 and child #2 were missing addresses for emergency contacts in the record.
Plan of Correction: Parent will provide address by end of day.
