Inspector Notes

An unannounced non-mandated inspection, required for a Provisional License, was conducted on 04/01/2024. The inspector was on site from 12:45pm until 3:50pm.

There were 50 children, ages 4 months to11 years old under the supervision of nine staff. Children were observed during indoor free play and nap time. Five children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on record review and interview, the center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the competition of a fingerprint background check as required by the Code of Virginia.

Evidence:
1.Staff #2, hire date of 1/23/2024 had fingerprint eligibility letter was dated 1/24/2024.
Plan of Correction: The center will ensure that the hire date of prospective employees does not become official until after full completion of background check.

REPEAT VIOLATIONS
Based on observation, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
On the school age playground, the top of the wooden boarder was separated from bottom boarder creating approximately a 3 inch gap. This causes a tripping hazard for children.
Plan of Correction: ? Corrective maintenance on playground equipment will occur on April 3, 2024.
