Inspector Notes

An unannounced monitoring inspection, resulting from a self-report received on 5/28/2024, was conducted on-site on 05/30/2024 and concluded on 06/10/2024. The inspector was on site from 3:40pm until 4:20pm.

Two children's records and one staff record were reviewed. The inspection included interviews with staff and review of documentation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations not related to the self-report were issues and are documented on the violation notice issued to the program.

When staff are supervising children, they shall always ensure their care, protection, and guidance. On 5/24/2024, Staff 1 engaged in inappropriate conversations with Child 1 (age 8) and Child 2 (age 9) about actions with another adult. During the conversations, Staff 1 included gestures describing the actions that were not appropriate for children.
