During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/22/2025, as a part of licensure period. The on-site inspection began at 9:50am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 34 children present and 8 staff. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included document review, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/3/2025. A POC submitted after this date will not appear on the public website.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

On the playground, there was a 3-inch drop from the asphalt to the mulch where the children play. This is a tripping hazard for children. Children were observed in this area.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

There were unlocked chemicals under the sink area in the twos room. The locking mechanism on the cabinet was broken

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall.

The fall zones on the large playground had un unmeasurable amount of surfacing under equipment.

When any surface has been contaminated with bodily fluids, it shall be cleaned and sanitized.

In the two-year-old room a child had previously vomited on the carpet. The staff used a product on the carpet to absorb bodily fluid. Once swept away, the carpet was not cleaned and sanitized. Some of the contents remained on the carpet. Children were observed in this area.

Medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.

In the medication box there was an epi pen that not in a box and did not have a label attached.
