An unannounced monitoring inspection was initiated on 12/4/2025 and completed on 12/12/2025 in response to a self-report received by the licensing office on 12/3/2025 relating to an injury. The inspector reviewed 1 child?s records and 2 staff records on-site. This inspection included document review, and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
