Inspector Notes

An unannounced monitoring inspection was conducted on August 30, 2022. At the time of entrance there were 45children and 12 staff members present with children. The sample size consisted of 4 children's records and 4 staff records. Children were observed eating lunch and preparing for snack . The inperson inspection began at approximately 10:40am and ended at approximately 12:10 pm.
Areas of non-compliance are identified on the violation notice.

Standard 8VAC20-780-270-A
Based on observation , the licensee did not ensure there was inside and outside , be maintained in a clean , safe and operable conditions.

Evidence:
There was a broken basketball back board by the entrance of the outdoor areas. There was broken equipment with sharp edges on the side of the red storage building. Children will pass the areas the broken equipment.
Plan of Correction: Director remved all broken equipment on 09/02/2022. THe back board will be removed by 09/12/022.
Standard 8VAC20-780-330-B
Based on observation , the licensee did not ensure resilent surfacing was loose filled synthetic materials meeting requirements of depth.

Evidence:
On the playground with the wooden playground equipment, the yellow slide had 2 inches of loose resilent surface ( pea gravel) and the swings had 4 inches of loose resilent surface (pea gravel).

On the little playground area, the tree with blue slide and green slide resilent surfacing was sand it was not a loose resilent surface. The resilent surfacing was unmeasurable becuase it was not loose.
Plan of Correction: Director has contacted the landscaper and is working to have more pea gravel and sand applied to and dispearsed accordingly. Spot checks will be done weekly to ensure resilent surfaced is maintained.

Based on interview and record review, the licensee did not ensure there was a monthly practice evacuation drill.

Evidence: The center did not document May , June and July evacuation drills.
Plan of Correction: Director has developed a new reminder system going forward.

Based on record review, the licensee did not ensure written record of children's injuries contained all required information.

Evidence:

Injury report for Child #1 was missing how parent was notified.
Injury report for Child #2 was missing how parent was notified and time and date notified.
