Inspector Notes

A renewal inspection was conducted on 8/22/2023. There were 56 children directly supervised by 11 staff members. Children were observed engaged in group activities; color chart and letter "C" art, blocks and outdoor play time. The physical plant, 4 child records, 4 staff records, medications, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-770-60-B
Based on a review of staff records, 1 staff has been employed without the documentation of completing the sworn statement.
Evidence: Staff #1, date of transfer 8/1/2023 does not have documentation of a completed sworn statement prior to working with children. The staff was observed caring for children this date.
Plan of Correction: Staff will complete the sworn statement and file in the staff's record.
Standard 8VAC20-780-240-A
Based on a review of staff records, 1 staff has not completed the required Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Evidence: Staff # 3, date of hire 3/28/2022 does not have documentation of completing the required 10 hour orientation course.
Plan of Correction: The staff will complete the 10 hour training and the certificate will be filed in the staff's record.
Standard 8VAC20-780-240-B
Based on a review of staff records, one staff has not completed orientation training no later than 7 days of the date of assuming job responsibilities.
Evidence: Staff #1, date of transfer 8/1/2023 does not have documentation of orientation training.
Plan of Correction: Staff will receive the proper orientation training and the training document will be filed in the staff's record.

Based on a review of children's records, the center has not maintained a record for each child that contains all the required documents.
Evidence: Child #1, date of enrollment 7/5/2021 does not have documentation of a completed physical and immunization record.
Child #2, date of enrollment 8/3/2020 does not have documentation of child's physical examination.
Plan of Correction: Management will request the physical and immunization records from the children, the documents will be filed in the children's record.
