During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, monitoring inspection was completed on June 12, 2025. The inspection began at 12: 20 PM and ended at 1:39 PM. The inspector reviewed compliance in the areas listed above. There were 24 children present and three staff. The inspector reviewed six children?s records and four staff records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/23/2025. A POC submitted after this date will not appear on the public website.

The child day center is required to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Staff #1 did not have documentation of out-of-state criminal records request, out-of- state child abuse and neglect registry search, and out-of-state sex offender registry search in their record.

For each group of children, the center is required to maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Classroom #1's attendance sheet had five children signed in but 4 children were physically present.. Classroom #2 had 8 children signed in but there were 10 children present.
