Inspector Notes

An unannounced monitoring inspection was conducted on 1/24/24 from 11:10am until 2:50pm. At the time of entrance, there were 60 children in care with 14 staff present. A sample of 6 children's records and 6 staff records were reviewed. Children were observed engaging in free play, participating in story time, eating lunch and resting quietly during nap time. Lunch service procedures, diapering procedures and restroom and handwashing procedures were also observed. A self-reported incident that occurred on 12/21/23 involving a possible injury to a child was also reviewed during this inspection. Areas of noncompliance are identified on the violation notice and were discussed with the center director at the conclusion of the inspection.

Based on record review and interview, the center did not ensure that the results of a criminal history record information check is obtained from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1 has a hire date of 8/23/23. Staff 1 indicated on her sworn statement or affirmation that she has resided in the state of Alabama within the past five years. The results of a criminal history record information check from the state of Alabama was not available for staff 1.
2. The center owner confirmed that the criminal history record information check for staff 1 from the state of Alabama has not been completed.
Plan of Correction: The center responded with the following: The center manager contacted the state of Alabama to obtain instructions on how to request the out-of-state criminal check. The request will be completed and mailed as soon as possible. Once the results are received, they will be forwarded to the licensing inspector and placed in staff 1's record. Going forward, center management will ensure that out-of-state background check are completed within the required timeframes.
Standard 8VAC20-780-140-A
Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.

Evidence:
1. The record for child 1 (date of enrollment: 9/10/23) does not contain documentation of a physical examination.
2. The center director confirmed that the record for child1 is lacking documentation of a physical examination.
Plan of Correction: The center responded with the following: Documentation of child 1's physical was obtained from the parent and placed in the child's record. Going forward, center management will ensure that documents and information needed for children's records are obtained within the required timeframes.
