During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/26/25. The on-site inspection began at 10:45am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 62 children present and 12 staff. The inspector reviewed 6 children's records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child 1, who had been in attendance for two days, did not have documentation of an immunization record on file.
Plan of Correction: The center responded with the following: The parent was contacted during the inspection and the child's immunization record was obtained and placed in the child's record. Going forward, center management will ensure immunization records are obtained by the child's first day of attendance.
Standard 8VAC20-780-280-B
Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

The door to a storage room, which contained numerous containers of cleaning chemicals and cans of latex paint on open shelves and the floor, was unlocked and assessable to children in care.
Plan of Correction: The center responded with the following: The door was locked immediately. Center staff will be reminded to ensure the storage room door is kept locked when not in use.
