Standard 22.1-289.031-A-4
Each person in a supervisory position has been annually certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #11 (substitute) has not provided a staff health report since 2022.
Plan of Correction: Staff #11 has been advised to update the staff health report as soon as possible. In the future the administration will ensure that all substitutes records are current.
Standard 22.1-289.031-B-5
The center's procedure regarding ensuring that all areas of the premises accessible to children are free of obvious injury hazards must be implemented. A basket of cleaners was in the hall, a Lysol Wipes were on the counter beside the outside door in the Camper room, and Lysol Wipes and Lysol spray were on the counter beside the sink used by children in the Caterpillar room. All were within reach of children.
Plan of Correction: All hazardous substances were moved to a location that is out of reach of children the same day as the inspection. Staff will be reminded to keep hazardous substances, including Lysol wipes, out of reach of children.

The center is required to have employees undergo a background check every five years. The most recent central registry check and sworn statement for staff #11 was overdue by a year and six months.
Plan of Correction: The central registry and sworn statement have been provided to staff #11 to complete. In the future the administration will ensure all substitute provider records are kept current.
