Violations

1
Standard 8VAC20-780-550-G
Based on record review, documentation of shelter in place and lockdown drills were not maintained.

Evidence:
1. The center only documented one shelter in place drill, which was dated 5/15/2022
2. The most recent lockdown drill documented by the center was dated 9/14/2021.
3. The center supervisor stated both the second shelter in place drill and lockdown drill were completed, but not filled out on the form used to document drills completed.
Plan of Correction: We will remember to write down drills when completed.
