Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/23/2024, as a part of the licensure period. The on-site inspection began at 2:06pm and ended at 3pm. The inspector reviewed compliance in the areas listed above. There were 77 children present and 14 staff. The inspector reviewed 5 children?s records and all staff records were reviewed at the Campagna main office on 6/3/2024. This inspection included: document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, observations (this would include observations of photos, recordings, etc.), and measurements
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/1/2024. A POC submitted after this date will not appear on the public website.

3The facility did not post the most recent inspection report. The report posted was dated 2/7/2024, but the most recent inspection occurred on 6/3/2024.
Observed bleach water in an unlocked cabinet, accessible to children in room 111.
In room 115 there were 16 children and two staff. The inspector observed one staff member leave the classroom with two children, leaving the other staff member with 14 children.
