Inspector Notes

A monitoring inspection was conducted on 6/8/2022 and concluded on 6/8/2022. There were a total of 17 children observed with 6 caregivers present. The children were engaged in the following: transitioning from outside time to meal time, washing hands, playing with balls, and diapering procedures. The inspector reviewed compliance in the areas of administration, personnel, physical environment, care of children, handwashing, diapering, emergencies and
nutrition. A total of 2 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Evidence:
Staff #3 (date of hire 5/30/2022) did not have a criminal history background check documented during the inspection.

Based on record review one of three staff files did not contain a child abuse and neglect registry finding or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #2 (date of hire 11/21/21) did not have documentation of a MD child abuse and neglect finding documented during the inspection.

Based on record review, one of three of the reviewed staff records did not contain the results of a Tuberculosis screening or test every two years from the date of the initial screening or testing, or more frequently as it is recommended by a licensed physician or the health department.
Evidence:
Staff #1 had a Tuberculosis screening or test dated 3/11/2020.
Plan of Correction: The employee went to get an updated TB screening/test.

Based on observation, prepared infant formula was not dated.
Evidence: Two of three sets of bottles were not dated.
Plan of Correction: We will make sure the bottles are dated when they get dropped off.
