Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 09/19/2024, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 2:50pm. The inspector reviewed compliance in the areas listed above. There were 21 children observed and 6 staff. The inspector reviewed 4 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-550-H
The center shall maintain a record of the dates of the practice drills for one year.
Evidence:
Documentation of drills completed prior to August 2024 were not available for review during the inspection.
Plan of Correction: The center will ensure a copy of the previous school year's drill logs are kept at the center.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the time when parents were notified.
Evidence:
One injury report reviewed did not include the time when the parents were notified.
Plan of Correction: The center will remind staff of requirements regarding documenting injuries.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
Evidence:
The records for Child #1, Child #2, and Child #4 were missing documentation of first dates of attendance.
Plan of Correction: The center will send an email reminding staff to add the start dates to all children's files.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include an authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds.
Evidence:
The authorization for emergency medical care for Child #1 was not signed by a parent.
Plan of Correction: The center will check all binders to ensure agreements are signed.
