Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/14/2025 and completed on 8/20/2025, as a part of the licensure period. The on-site inspection began at 11:01AM and ended at 12:39PM. The inspector reviewed compliance in the areas listed above. There were 8 children present, 2 caregivers and 1 household member; the point total was 16. The inspector reviewed 3 children?s records and 1 caregiver record on-site on 8/14/2025.

Standard 8VAC20-800-60-B
Children's records are required to contain the following information: The record for Child #2 was missing 9 required components. The child was present during the inspection and had been in care for 5 months. Child #3 was missing first date of attendance and was present during the inspection.
Plan of Correction: Provider will ensure childrens records are available at all times.
Standard 8VAC20-800-70-B
Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration. There was no parent signature for the liability insurance declaration for Child #2 who was in care for 5 months.
Plan of Correction: Provider will ensure childrens records have information for parents.
