Inspector Notes
A renewal inspection was initiated and concluded on 11/16/2022. There were a total of 48 children and 16 staff members present. Children were observed resting, transitioning from resting, circle time, gross motor skills, and outdoor play. Staff were actively engaged and interactive with the children.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

2Evidence:
1. Staff #1 (date of employment 4/14/2008) tuberculosis screening expired on 7/27/2022.
Evidence:
1. In the classroom for two and three year old children, the toilets in the classroom had protruding nails that did not have caps to cover the nails.
2. Seven mini blocks in the toy area had rips in the corner.
3. In the school-age classroom, the red couch had tears on the side.
We will wrap tape for cover and contact facilities for replacement.
